Grantee Spotlight: Project H.O.P.E.
For many patients with a chronic disease, learning about proper nutrition and how to cook nutritious meals is a key to maintaining one’s health. The CVS Health Foundation and the National Association of Community Health Centers (NACHC) are proud to provide an “Innovations in Community Health” grant to Project H.O.P.E in Camden, NJ, for their intensive Chronic Disease Management program. Through the grant, Project H.O.P.E provides nutrition education and cooking classes to individuals who are homeless or at-risk of being homeless, in collaboration with Cathedral Kitchen, the largest soup kitchen in Southern New Jersey.

North Country HealthCare - Flagstaff, AZ
This program will proactively identify patients admitted into the hospital and staff will develop care-transition plans, thus decreasing the number of hospital readmissions. Learn more

An Integrated Chronic Disease Team (ICDT) will focus on 400 patients with diabetes and/or hypertension, to help them manage and control their conditions. Goals are to improve quality of life and decrease hospitalizations and complications. Learn more
