Speaker 1: By having the $1 million grant, we're able to provide more patients with quality health care in the state of Rhode Island without leaving the house. Or we can go into their communities if there's any transportation issues, to make sure that they're not getting missed.

Speaker 2: And what we did is create a program that is based on a nurse practitioner and certified community health worker who meets every single patient after their babies are born to have high blood pressure and their postpartum rooms, gives them a blood pressure cuff and then follows them remotely, allowing them to take their blood pressure at home, keeping them safe, but also with their babies.

Speaker 3: Having this great grant from CVS will help us expand our program and be provided with quality care from their houses, which diminishes the issues of having gaps in care that’s actually contributing for their safety.
