Aetna Announces New Accountable Care Agreement in Georgia with Sequent Health Physician Partners

ATLANTA, Georgia – Aetna and Sequent Health Physician Partners today announced a new accountable care agreement that is designed to improve the quality of care, enrich the consumer experience and reduce health care costs for the Aetna members in metro Atlanta.

Aetna members who receive care from the 225 physicians at Sequent Health Physician Partners in Gwinnett County will be able to access more coordinated care, particularly those patients with chronic or complex conditions. Members will also benefit from the improved flow of information among treating physicians within the network. Sequent Health Physician Partners is a working partnership between a network of community physicians and Gwinnett Medical Center. There are approximately 11,000 active Aetna members in the network who will benefit from this agreement.

Aetna’s accountable care collaboration with Sequent Health Physician Partners will feature a new model of health care delivery designed to offer the following benefits to consumers:

  • enhanced coordination of care between doctors and other care providers;
  • an improved patient experience through high quality care management programs;
  • use of advanced technology to help providers care for their patients;
  • lower overall costs for members who see providers that are part of the Sequent Health Physician Partners network; and
  • better health care outcomes for patients.
