Who is eligible to use this app?
This app is available for Aetna and Caremark members who are enrolled in the Transform Diabetes Care program. Members in select plans may not be eligible yet, but we are continually working to expand access.
Is there a cost associated with this application?
CVS Health Tracker is provided at no cost to eligible Aetna and Caremark members. It is being provided to assist members in managing their health.
Is this application a requirement for members, or something they can voluntarily participate in?
This application is not required and the application's use is completely voluntary.
I am having issues with the application and cannot figure it out. Who can help me resolve my issue?
If you are an Aetna member, call the number on your Aetna Member ID card. If you are a Caremark member, please contact us at (855) 208-4858.

The app currently supports the following glucometers:
