Oak Street Health advances primary care for older adults
Oak Street Health® is a network of primary care centers dedicated to improving health outcomes for older adults, particularly those in underserved communities. Its innovative care model integrates physical, behavioral and social health services to provide comprehensive, patient-centered care. Oak Street Health empowers patients — most of whom are Medicare beneficiaries averaging 69 years of age and managing multiple medications — to take control of their well-being. Through ongoing engagement, screenings and social activities, Oak Street Health helps patients stay healthier and avoid unnecessary hospital visits.

Signify Health supports healthy aging at home
Signify Health® In-Home Health Evaluations (IHEs) offer eligible health plan members a detailed picture of their health and provide personalized steps to stay healthy — right at home. Many Medicare Advantage plans cover IHEs for their members, which can help bridge gaps in care for older adults. During a visit, Signify’s experienced clinicians spend up to an hour with members to assess physical and behavioral health, review medications, screen for chronic conditions and assess social determinants of health. For older adults, these evaluations are especially valuable for identifying potential risks like falls, social isolation, cognitive decline, and unmanaged chronic conditions such as diabetes and heart disease.
Signify clinicians can collect over 300 clinical and social data points about a member’s health during an in-home visit. After the visit, a summary is shared with the member, their primary care provider and health plan to coordinate care. When appropriate, Signify also provides personalized follow-up support — whether that’s finding a primary care provider, scheduling a vaccine appointment or providing additional testing to better manage certain conditions.
MinuteClinic uses the “4Ms” framework for older adult care
In 2020, MinuteClinic® was recognized as a participant in Age-Friendly Health Systems, an initiative of the John A. Hartford Foundation and the Institute for Healthcare Innovation, by implementing the “4Ms” framework into its care for older adults. This framework focuses on four key areas essential to older adult health: What Matters, Medications, Mentation (including prevention and management of depression and dementia), and Mobility.
As the largest retail clinic network in the U.S. to adopt this model system-wide, MinuteClinic is helping older adults receive personalized, proactive care in settings that are both accessible and familiar — including through virtual visits from the comfort of home. Between 2021 and August of 2025:
- More than 6,000 older adult patients with potential mobility concerns have been identified and given guidance to ensure safe mobility and help prevent falls.
- 342,240 older adults received medication screening. This includes screening for potentially harmful medications based on the 2025 American Geriatric Society Beers Criteria® list.
- Over 145,000 older adults were identified as taking medications (prescribed and over the counter) that may cause avoidable harm and were supported in making safe changes or taking alternative medications to minimize risk, in collaboration with the prescribing provider.
- 422,139 older adults have received depression screenings.
In 2022, MinuteClinic was further recognized as a “Committed to Care Excellence” Age-Friendly Health System.
