New plan provides access to coordinated, high-quality care with a $27 monthly plan premium
FAYETTEVILLE, N.C. — Aetna today announced a new Aetna Medicare Advantage preferred provider organization (PPO) plan. This plan has a $27 monthly plan premium for Medicare beneficiaries in the Sandhills area, including Chatham, Cumberland, Harnett, Lee, Moore and Sampson counties. The Aetna Medicare Value℠ Plan (PPO) will provide access to the Community Care Physician Network, LLC (CCPN).
The CCPN physicians and other health care providers that are covered at in-network rates include:
- Clinton Medical Center;
- Moore Family Medicine;
- Lillington Family Medical Center;
- StatCare Urgent and Primary Care;
- Fayetteville Internal Medicine;
- Village Family Care Center, PA;
- Cumberland Internal Medicine; and
- Southern Regional Area Health Education Center (AHEC).
CCPN patients, especially those with chronic or complex health needs, who enroll in this Medicare Advantage plan will experience more coordinated care. They’ll benefit from improved information flow to the CCPN physicians providing care. Aetna nurse case managers will assist CCPN care providers with care coordination, outreach and follow-up services.
Plan highlights include a $0 copay for primary care physician office visits, preferred generic drugs at preferred pharmacies and routine vision and eye exams, along with $0 medical annual deductible. Members also have access to a SilverSneakers® gym membership at no extra cost.

This plan will be available in the Sandhills area, including Chatham, Cumberland, Harnett, Lee, Moore and Sampson counties. You can enroll during the Medicare Annual Enrollment Period from October 15 to December 7. The effective date of the plan will be January 1, 2018.
