On the first day of the Valleywise Diabetes Education Empowerment Program (DEEP) in Phoenix, Arizona, a determined woman entered the room hoping to find ways to manage her type 2 diabetes. This chronic condition had not only challenged her mobility but dimmed her eyesight. As the program unfolded, she courageously shared her health journey and personal story, which deeply resonated with fellow participants. Her openness helped create a strong bond among those navigating similar struggles, nurturing a supportive community within the group and, ultimately, making it easier for her to manage her chronic condition.
What set her apart was her support system, including Oscar Solis, a patient navigator for the Valleywise Health DEEP program who played an invaluable role in uplifting and inspiring her to manage her type 2 diabetes better.
“Understanding that managing type 2 diabetes can be complex, program coordinators work to build personal connections with each participant,” said Oscar Solis, Patient Navigator at Valleywise Health. “We are committed to delivering care with genuine kindness and compassion, which is ingrained in our culture.
