With customized communications and a broad set of channels, CVS Health supports more tailored, effective diabetes care. By meeting members wherever they are in their health journeys, we can help support positive health outcomes and reduce overall costs. Transform Diabetes Care leverages technology across three key steps:
1. Identifying gaps in care
We review claims, lab results and other data in an effort to create a comprehensive view of each program member. This allows us to identify potential gaps in care and make sure that members receive support aligned to their specific needs and health goals. CVS Health has found that nearly 73% of members with diabetes identified for intervention have at least two gaps in care.*
For example, data analysis may show that a member has not kept up with annual screenings — an important step for reducing the risk of developing a more complex condition and comorbidities.
2. Customized outreach
We use analytics to help identify next-best actions for each member. Different levels of outreach are recommended based on the severity of the member’s condition, then members receive targeted nudges with information about gaps in their current care or next steps.
For example, multiple out-of-range readings from a member’s digitally-connected glucometer may prompt outreach from a nurse to discuss next steps to help them get back on track.
3. Member impact
We implement steps designed to monitor member results and measure improved health outcomes for individuals. Unlike more limited typical approaches, our program works to track health outcomes across five clinical areas:
- Blood glucose monitoring

This personalized approach can help improve outcomes for members with diabetes. Lowering A1C levels and systolic blood pressure in people with diabetes helps control blood sugar and reduces the risk of serious complications like heart disease, kidney damage and nerve problems.
