Improving health one In-Home Health Evaluation at a time

We've been married fifty eight years. When John first got diabetes, it was a change in our lifestyle a little bit. And then with his Parkinsonisms and his lack of balance, he's really relegated to a chair or wheelchair.

So that's changed our lives a little bit.

I think with fifty eight years, it's good to hear each other tell someone else what's what their concerns are because we don't we're kind of deaf to that sometimes.

With Signify Health, I really like the fact that my husband and I see them together and they spend time with us and talking about our general well-being rather than when we go to a doctor, he's focused on some specific issue.

One of the best visits I had was one of my first visits. Nurse practitioner that came to do it, I talked about being depressed, and she helped me understand that I was and helped me go to my doctor and then get the right medicines for it.

By hearing us directly answer someone else about our health, it helped us understand what each one of us was going through.

One thing that's led to our ability to stay married for fifty eight years is our ability to be patient with one another. Definitely. And pay attention when you're when she's saying something to me about what's going on with her.

Don't blow it off.

Creating personalized experiences – where people feel heard and valued – is key to building trust in health care. And nowhere is more personal to someone than their home. 

Through In-Home Health Evaluations (IHEs) from Signify Health, experienced clinicians visit eligible health plan members in their homes with a goal to help keep members healthy and connected to care.

By working with Medicare Advantage, Medicaid and Commercial health plans, Signify’s in-home health visits support better outcomes and engagement for health plans and members alike, from performing preventive screenings to identifying health risks earlier. 

Identifying health risks and building a full picture of member health

During IHEs, Signify’s experienced clinicians – physicians, nurse practitioners and physician assistants – assess plan members’ overall well-being. As part of the visit, clinicians conduct medication reviews, screen for chronic conditions, assess mental health and work with members to create a plan that helps them better manage their care.

IHEs often glean insights that may be missed during a traditional doctor’s office visit, like safety risks within the home. For example, clinicians evaluate and can help mitigate risks for falls, the leading cause of injury-related death for Americans over 65.

Clinicians can capture more than 300 social and clinical data points during an IHE.

As part of Signify Health’s Diagnostic and Preventive Services portfolio, clinicians may also perform diagnostic tests and screenings that can identify if someone is at risk for common chronic conditions or complications. This may include drawing blood to test blood sugar levels, performing eye exams that can identify signs of complications from diabetes, and administering cognitive screenings.

After an IHE is completed, a summary of the visit is shared with the member, as well as their health plan and primary care provider, so that they can develop a personal health management plan. By capturing clinical, behavioral and social insights in the home, plans can identify risk earlier, prioritize outreach and support more effective follow-up care.

We’re focused on guiding members toward better health while lowering costs for our clients and the overall health system.

Using in-home health assessments to connect members back to care

Signify also helps members re-engage with the health care system. Among those who have benefitted from IHEs are Carol and John, a married couple of nearly 60 years who have supported each other through many of life’s changes and transitions. John was first diagnosed with diabetes and then with Parkinsonism syndrome, leaving him reliant on a wheelchair.

During a recent IHE, a nurse practitioner learned that Carol was experiencing emotional and mental challenges related to her caregiving responsibilities. The nurse practitioner worked with Carol so she was prepared to speak about these mental health needs with her primary care provider, so she could get the support she needed.

Based on the trust our clinicians build with members during the In-Home Health Evaluation, 70% of members are receptive to additional care coordination.


With enhanced care coordination, the goal is simple: use information to make people healthier. Care coordinators translate insights from in-home health visits into personalized, next steps for members.  This coordinated action is critical for members with complex, chronic or fragmented care journeys, where missed follow-ups can result in avoidable complications and cost. For example, if a health plan member does not have a primary care provider, care coordinators will help them find a provider in their network and schedule an appointment. Additionally, clinicians can also help connect members telephonically to their health plan’s care management resources.

Timely care gap closure and care coordination support improved quality performance (impacting Star Ratings) while creating a tailored experience for members.

Enabling value-based care

As part of CVS Health Care Delivery, Signify Health enables value-based care models by helping providers and payers identify risk earlier, close care gaps and engage members in timely follow-up care.

Value-based care relies on a strong foundation of member and patient population data points, such as those collected through Signify’s in-home health visits. Equipped with rich data sets, providers and health plans can better understand member needs, assess risk accurately and allocate resources more effectively. In 2025, Signify Health performed more than 3.5 million In-Home Health Evaluations, impacting more than 16 million quality care gaps.
